Recognition

If you would like to request a Certificate of Congressional Recognition for a member of Arizona’s First Congressional District, please fill out and submit the online form below.  You can also print the form and mail it to my District Office, in Scottsdale.  The categories of recognition are below.    

 

Military Service Recognition: This certificate is presented to individuals in AZ-01 who are currently an active soldier or a veteran and have strove to improve the lives of others. The purpose of this recognition is to honor Veterans and current service members who have performed good works to improve our community. It is recommended you share a copy of the veteran’s DD214 with my district office.

Examples: A current service member actively volunteering for a veteran’s home, veteran who frequently volunteers at a non-profit, veteran or active service member who has completed a feat of heroism in our community

 

Educational Achievement Recognition: This certificate is presented to students and teachers who have excelled in their academic career or service to their school and community.  The purpose of this recognition is to honor students and teachers who have worked hard to advance their own academic success and improve the lives of those around them.

Examples: U.S. Presidential Scholar, Valedictorian, Teacher of the Year, Representing AZ-01 in state or regional competition, Appointment to one of the Military Academies, School Sports Coach of the year, Deans/Presidents/Principal’s list for above average GPA

 

First Responder Recognition: This certificate is presented to individuals in AZ-01 who put the wellbeing of others before their own to protect and serve our community.  The purpose of this recognition is to honor the First Responders who continually sacrifice their own safety for the betterment of others. 

Examples: Retirement from the force, community service outside of normal duties, feats of heroism, awards for service

 

Community Achievement Recognition: This certificate is presented to an individual or individuals, non-profit, church or charity who have been awarded for their achievements, or who go above and beyond for members of AZ-01.  The purpose of this recognition is to honor residents of our community who work and strive to improve the lives of those in need.    

Examples: Awards recognizing charity achievement, good Samaritan in the community, successful fundraisers, artists who are featured

 

Business Achievement Recognition: This certificate is presented to businesses who have reached an important milestone, have done charitable works in our community, or have gone to great lengths to support their employees. The purpose of this is to recognize businesses within Arizona’s First Congressional District who have gained success and have worked to improve the lives of others.

Examples: 1st/5th/10th anniversary of a business, business being recognized “Healthiest Place to Work”, business being named one of Arizona’s top 10 restaurants, and community service efforts or donations.

    What are these options?

    Constituents who are hard of hearing or use a video phone have the option to choose TDD or VP based on the type of device they are using. This allows our office to respond to them accordingly. The default option “Voice” is a standard audible telephone.
